If your GP surgery is allowing online appointments, you will be able to use Amity to book your next appointment online, as well as being able to review any past or upcoming appointments.
From Amity, select the 'My Health' tab and choose the 'Appointments' option.
From here you can see any upcoming and past appointments and you can also book or cancel an appointment with your GP surgery.
To book or cancel an appointment, select 'Book/ Cancel'.
You will be directed to the Evergreen Life website to manage your GP surgery appointments.
If your surgery is allowing online appointments, you will be able to view and book available appointments and cancel or change any that you are unable to attend.
You can return to Amity by selecting the option 'click here to return to your Amity overview page'.
Your appointment information will now be visible from within Amity.
