Health questionnaires provide a useful measure for gauging the effectiveness of care services and treatment.
From within Amity, you can view questionnaires sent to you from within the 'My Health' tab.
From Amity, select the 'My Health' tab and choose the 'Questionnaires' option. A green feedback bubble will be visible next to this option indicating the number of questionnaires requiring a response.
Once selected, a questionnaire dashboard will be visible. If no questionnaires have been sent to you, the dashboard will be empty.
If a questionnaire has been sent to you, select the questionnaire and choose 'Start Questionnaire'.
Go through the questions, selecting the appropriate answers.
Once complete, you can either save a draft of the questionnaire or choose 'Done' to submit your answers.
