From Amity, you can view a summary of your GP health record and you can provide additional information about your health.
Your GP health record shows personal data, such as your details, allergies, and medications.
Depending on what your GP surgery shares, you may also be able to see a more detailed medical history, including problems and consultation notes, as well as test results you may not have discussed with your doctor.
Important: Your record may contain sensitive information. If someone is pressuring you for this information, please contact your GP surgery immediately. You have a legal right to access the information in your record.
You can supplement the information provided by your GP surgery by providing additional information about your health such as lifestyle information, other medications being taken, or allergies.
From Amity, select the 'My Health' tab and choose 'About my health'.
From here you can review the information that has been recorded in your summary health record by clinicians at your GP surgery and the information you have chosen to record in Amity.
